Program Objectives:

  • To provide 11 permanent subsidized newly renovated one bedroom apartments (3handicapp accessible) to homeless individuals and couples
  • To provide 24 hour seven day a week coverage and access to mental health professionals, drug and alcohol treatment and intensive case management and employment specialist
  • To provide one on one life skills training
  • To provide access to community resources
  • To create a supportive and nurturing environment

  • Must be able to live independently
  • Must be chemically free

How Does the Program Generally Work:

People are referred by emergency shelters and other social service organizations. Phone intakes are conducted to identify client’s eligibility. Clients are then invited to a interview with representatives of ACTION- Housing, Inc. and Turtle Creek Valley MH/MR.

Program Purpose:

The Homeless families Service Program was initiated to provide services for families leaving emergency shelter and bridge housing programs and experiencing other housing crises. The program goals are to assist participating families in finding safe, decent and affordable housing as well as strengthen personal awareness, family cohesion and the utilization of community resources.

How Does the Program Generally Work:

People are referred by emergency shelters and other social service organizations. Phone interviews are conducted to identify clients. Clients are then provided with housing information and applications for housing. Resources used include Supportive Housing Management Services, public housing communities and other providers

Eligibility Criteria/Clientele:

To receive services through the Homeless Families Service Program families must be experiencing a housing crisis or residing in a shelter or bridge housing facility.

Program Purpose:

The purpose of MyPlace is to provide transitional housing to homeless young adults who have aged out of the foster care network in Allegheny County. MyPlace will allow young adults to learn how to live independently and become self-sufficient in their own apartment by working on goals related to employment, education, and life skills as well as creating community connections and life long relationships.

How Does the Program Generally Work:

MyPlace places program participants in their own one-bedroom apartment in Allegheny County and provides in depth case management to ensure all participants have the necessary resources to become self-sufficient. Participants can stay in their MyPlace apartment for two years while they work toward their goals.

Eligibility Criteria/Clientele:

To be eligible for MyPlace, a person must be between the ages of 18-24, and be homeless or about to become homeless. Also, a person must show an ability to hold employment as well as live independently in their own apartment. The target population for MyPlace is young adults who have aged out of the foster care system in Allegheny County.

Specific Benefits/Services Provided:

In addition to safe and affordable housing, MyPlace participants receive intensive case management. Case management includes weekly face to face meetings, goal planning and referrals to other services necessary for participant success, as well as group sessions devoted to life skills and other important issues facing young people.

Application/Filing Procedures:

Application and referral forms are necessary to be considered for MyPlace. The application form can be found here and the referral form here. Each applicant will also need to provide specific documentation found here before an interview may take place. The referral form must be filled out by a staff person at an agency with a history of working with the individual who is applying. Both the application and referral must be mailed to:

Program Purpose:

The role of the Social Service Coordinators is to coordinate supportive services for the elderly, disabled and families residing in eligible housing projects. Service coordinators link the elderly or disabled residents residing in the projects to the supportive services necessary for them to remain independent and in their own homes.

How Does the Program Generally Work:

Supportive Services initiatives occur in many ways. Property Managers, residents, residents' families and various agency personnel are several of the sources or referral for supportive services. Coordinators regularly perform Supportive Service information gathering with residents however, the service is voluntary and is not meant in any way to be intrusive or interfere with the private and independent living choices of residents. Frequent communications is considered essential. Through periodic newsletters, Tenant Counsel meetings, and regular office hours, education and accessibility to supportive services is increased.

Eligibility Criteria/Clientele:

The eligibility for supportive services is determined thought the HUD Grant Agreement, which funds the Social Service Coordinator Program. The following properties are included in the Fund Agreeement: Crafton towers, Allegheny Independence House, Independent Housing Resources East (four properties), and Versailles/Archer Place. All tenants residing in these projects are eligible for Supportive Service Coordination.

Specific Benefits/Services Provided:

The Coordinators arrange educational meetings; act as liaison between residents and building management; assist with completion of applications for PACE, Medical Assistance, LIHEAP, etc.; identify residents with unmet needs and help them access available services; work with tenant associations and coordinate with outside service support workers who will perform manual tasks of daily living such as shopping, personal care and laundry assistance.

Application/Filing Procedures:

No application is necessary. A resident survey is conducted at the initial point of contact with the tenant. Files on each tenant are maintained and kept confidential.
